TVDCTA Volunteer Rewards Application
If you have completed at least four hours of volunteer
hours for the club and would like to redeem your $25.00
TVDCTA Rewards, please complete the form below with all of the information
and give to the Volunteer Coordinator for verification.
The Treasurer will send a check to the address you provide below.
Remember, the rewards are $25 per year, per member and volunteer hours
cannot be carried from one year to the next; however, immediate family members
hours can be counted towards required hours.
The requirements are:
* You must be a TVDCTA member in good standing and your membership must be paid for the year.
* Four or more hours of volunteer service at a show or clinic sponsored by TVDCTA
* The $25 is a ‘reimbursement’ for entry/membership renewal fees you have already paid.
